Please arrange an appointment for a travel consultation at least 6-8 weeks before departure if possible. Vaccines need time to take effect and some may require a course over several weeks.

Please attend the surgery to collect a travel vaccination form. Alternatively, you can click on the link below to download the form. Once completed please hand in to the surgery. Once a nurse has reviewed your form they will contact you advising you what to do. You may be advised to attend a private travel clinic if we cannot see you before you go or if you need specific vaccines that we do not asminister such as yellow fever.
